Acknowledgement Waiver & Release:

The undersigned hereby acknowledges that he/she has voluntarily chosen to participate in an event sponsored by and called the Raleigh Pink Ribbon Ride for the benefit of 1ofUs. This event will involve an activity that could potentially be dangerous and harmful; and the undersigned has chosen to participate in this event with the full knowledge of his/her potential danger and harm. By signing this document, the undersigned represents and warrants that he/she does not have any physical condition which would be aggravated or worsened by extreme physical stress. If you have any such condition, DO NOT PARTICIPATE IN THIS EVENT. In consideration of the Raleigh Pink Ribbon Ride and 1ofUs permitting the undersigned waives any claim for personal injuries against Raleigh Pink Ribbon Ride, it’s sponsor and 1ofUs arising out of, or in any way related to the undersigned’s participation in this event, and expressly release Raleigh Pink Ribbon Ride and 1ofUs from any and all claims for personal injuries arising out of or related to participation in this event. IF YOU DO NOT INTEND TO COMPLETELY WAIVE AND RELEASE ALL CLAIMS AS DESCRIBED, DO NOT PARTICIPATE IN THIS EVENT.
